Change to maiden name (i.e. after divorce)

However, there are several instances in which the application will NOT be approved, including:
Spelling to include numbers or punctuation
To contain obscene words or racial slurs

To successfully change names in California, there are several steps to take, and procedures may slightly vary from county to county. Our team at Rinne Legal would be happy to help with this general guideline to achieve a new name:
Fill out forms with accuracy and to completion (CLETS Background Information Form, NC-100, NC-110, NC-120, CM-010, NC-130)
Make a copy of each completed form
File in applicant’s home county (typically Probate Court, sometimes Civil or Family Court)
Publish the Order to Show Cause form with an approved Newspaper Publishing company once a week for 4 weeks prior to the court hearing. (exclusions apply: name change to conform to gender identity, name change as a result from stalking, sexual assault or domestic violence)
Take the Order to Show Cause (NC-120) and Decree Name Change (NC-130) form to your Court hearing
If the Judge signs the Order to Show Cause, get a certified copy made for your records and use it as proof to change all government-issued Identification such as Social Security, Passports, Birth Certificates, Driver’s License, etc.
